Disease Risk Reduction 2009 Grant Application: Harvey County
Logic Model
Goal
1: Preventing Initiation of Tobacco Use among Young People
Inputs:
Grant Manager, School Health Councils, Regional Prevention
Center, Communities in Schools, Tobacco Workgroup.
Activities:
Recruit youth for the Tobacco Workgroup; provide logistical
support for teens to attend tobacco-related events, provide
advocacy training and logistical/technical/material support
for teen-led counter-marketing efforts.
Outcomes:
Short-term:
Educate teens on the health risks of tobacco use, recruit
teens as advocates for counter-marketing among their peers.
Intermediate:
Comprehensive Plan developed and adopted by the Harvey County
Coalition; reduce tobacco use initiation by young people.
Long-term:
Reduced risk factors for future chronic disease in current
teens; reduce disparities for ethnic minority teens, reduce
morbidity in adulthood for current teens.
Goal
2: Eliminating Exposure to Secondhand Smoke
Inputs:
Coalition Members, Grant Manager, Outreach Coordinator for
TUPP, TFKC
Activities:
Determine level of public education necessary. Produce a strategic
plan for community education efforts in the three cities without
an ordinance. Identify city council members and survey key
members to assess support for the ordinance.
Outputs:
Short
Term: Plan constructed for introduction of the ordinance.
Resources and support identified. Media plan designed. Local
support in place.
Intermediate:
Comprehensive Plan developed and adopted by the Harvey County
Coalition; increased support for creation and active enforcement
of tobacco-free /Clean Indoor Air policies.
Long-term:
All Harvey County citizens protected at work and in public
by smoke-free policies, reduced exposure to secondhand smoke,
reduced rates of chronic diseases and morbidity associated
with tobacco use or exposure.
Goal
3: Promoting Quitting Among Adults and Young People
Inputs:
Grant Manager, Health Department staff, Health Care Professionals,
TUPP Cessation Manager, Hispanic Council of Harvey County,
Interpreter for HD, TUPP special population’s staff,
Faith Community council, Harvey County Capacity Building Partnership.
Activities:
Provide training opportunities for health care professionals
to provide the 5A’s for tobacco cessation and to promote
the Kansas Quitline in Harvey County. Utilize media opportunities
on local radio and print ads to promote the Quitline. Conduct
focus group discussions to create a plan for addressing Hispanic
populations in Harvey County in health promotions activities.
Outcomes:
Short
term: Increased number of providers following tobacco
use screening and referral guidelines for Quitline.
Intermediate:
Comprehensive Plan developed and adopted by the Harvey County
Coalition; increased number of quit attempts, reduce tobacco
use; ongoing identification of tobacco-related disparities;
dissemination of culturally competent interventions.
Long-term:
Reduce secondhand smoke exposure, chronic disease, morbidity,
and decrease disparities associated with the use and effects
of tobacco.
