Disease
The
Health Department is required by law to investigate cases
of reportable disease.

Notifiable Disease Form

Laboratories and physicians contact the Health Department
about any positive results for “reportable”
diseases.

Upon receipt of a case report of a reportable disease, the
Health Department will investigate the case by contacting
the person(s) affect and / or the healthcare provider treating
the case(s). Case investigations may also require gathering
information from and notifying the contacts of the case.

Information gathered during a reportable disease investigation
is reported to the Kansas Department of Health and Environment
(KDHE) via a secure computer network. The Health Department
is required by law to follow HIPPA privacy regulations regarding
a case’s personal health information.

Disease educators are available to provide information to
individuals affected by the disease outbreak.
