CAB CARD
To obtain your new cab card, these are the items needed:
- Completed application form.
- Proof of income.
- A letter of disability from your medical doctor or an SSDI statement, if under 60 years of age.
This information needs to be returned to Transportation, 800 N Main St, PO Box 687, Newton, KS 67114. If all qualifications are met, we will mail your cab card.

Green Card: Income below 125% of poverty level (1) person cost $2.50 per ride
Red Card: Income between 125% and 185% of poverty level (1) person cost $3.25 per ride.
